Initial Controls

Activity: Internal cleaning for piping: 

1.Preparation and hauling of pipes, tools and equipment prior to start of work.

1.1 Sharp edges of tools and equipment that are not easily spotted. • Secure appropriate work permit prior to start of work.
• Conduct toolbox talk and supervisor to discuss the hazard and control measure indicated on the TSTI prior to start of work.
• Provide guards to sharp edges of all equipment.
• Install barricade tapes provide warning signs on sharp materials to make it visible for all employees present in the work site.
• Provide proper personal protective equipment i.e, hard hat, safety shoes, safety glass and working gloves.
• Impose staggered working hours. Include substitution of workforce to allow sufficient rest in between employees.
• Provision of cool drinking water and shaded rest place industrial fan.
• Supervisor shall monitor their work force by looking for signs of dehydration.
1.3 Severe Weather Condition (Sandstorm, High winds, etc.) • Secure all loose objects and materials (i.e. Ladders, pallets, portable toilets, Gl Sheets, drums, trash in dumpster, etc.)
• Set brakes and secure all mobile equipment / vehicles.
• De-energize all unnecessary electrical power.
• Appropriate material should be available and stored on the project to facilitate the securing of project property, materials and equipment, i.e nylon rope, wire, tarpaulins,
boards / plywood, etc. 
1.4 Poor manual handling techniques • Maintain proper back posture and body position during manual lifting of materials. Position body where less effort is exerted and less strain.
• Use crane, forklift, or manual lifters for heavy loads.• Divide load weight between co-workers.
• Assign additional person at the trailer bed to assist for loading/arranging small bore pipe spool.
• Assign sufficient numbers of personnel who could sustain the load when doing manual handling.
• Observed proper communication between coworkers during execution of specified task.
1.5 Formation of fumes in confined space area may lead to suffocation. • Forced ventilation shall be arranged wherever work is to be carried out in a confined space area.
• Gas analysis should be conducted prior and during activity by HSE department.
• Electric cables at least 7 ft in height or provide protective guarding if to be laid on the ground. use metal hooks protected by rubber hose.
• Observed proper housekeeping and material management when the work is done.
• Workers shall be equipped with proper personal protective equipments (PPE’s). 
1.6 Sub standard defective electrical facilities • Do not use dilapidated cables and tampered outlets.
• Observed proper bonding of materials and equipments and install sufficient grounding for generator.
1. 7 Poor practices on maneuvering heavy equipment. • Heavy equipment  operators/drivers must undergo competency training and certification before operation/driving heavy equipment.
• Equipment should have
blinker light/sound.
• Provide PWAS to equipment.
• Assign banksman or signal man to direct maneuver of heavy equipments.
• Install barricade tapes and provide warning signs to.
2. Water cleaning 2.1 Failure of equipment
2.2 Loose connections and unsupported pipe lines.2.3 Water not drained properly.2.4 Environmental impact (Water contamination).
• Ensure that the equipment used for the rest has been inspected and has a valid  inspection tag and color code.
• Ensure that the area is cordoned with barricade and warning signs.
• Ensure that proper PPE are provided for all personnel involved in the activity.
• Ensure that all connections are properly tightened and all pipe supports and hangers are completely installed prior to start flushing.
• Ensure whiplash arrestors are properly installed.
• Assign competent personnel for the job.
• Ensure that proper PPE’s are provided to all personnel involved in the activity.
• Cover all electrical equipment in the area.
• Ensure that drain system can cope with the volume of water.
• Disposal of the effluent should be in the holding tank and must be adequate to avoid flooding.
• Isolate the area with barricade and signs to prevent unauthorized entry.• Discharge of waste water should be done by certified waste water collector.
• In case of flange breaking, draining a lve shall be connected with hose to avoid shower.
3. Air Blowing. 3.1 Failure of equipment i.e Air Compressor • Equipment used bearing with current inspection sticker and color code for t he specific month.
• Qualified inspector shall be assign to conduct through inspection.
• Ensure that equipment equipped with fire extinguisher, properly grounded and provided with drip tray. 
3.2 Unsafe practices or incompetent of personnel performing the  job.

3.3 High intensity noise /sound.

• Provide whiplash arrestor on hoses which are possible to disconnect or where high air pressure is present.
• Ensure work area is provided with corresponding “Air Blowing in progress”.
• Ensure that comrding “petent person will authorized in the job and undergo t raining regarding “Hydro / Pneumatic Training”
• Personnel performing air blowing shall wear appropriate personal protective equipments (PPE’s) including face shield and reflective high visibility vest at all times.• Provision of air nozzle shall be sued.
• Provide signages pertaining on ear muffs / plug required beyond this point.
• Personnel conducting air blowing shall be equipped with ear muffs I plug at all times.
4. Air / Nitrogen cleaning 4.1 Eye irritation, loose of hearing and physical injuries
4.2 Slips, trips and fall
4.3 Over exposure, asphyxiation.
• Secure approved work permit prior to start of work.
• Ensure blow point was isolated with barricade, post warning signs and standby watchman. • TSTI must be conducted and discussed by the job In-charge / Supervisor and understood by the workers.
• All personnel involved shall wear clear glass and additional personal protective equipment like face shield, ear muff or ear plug.
• Walkways must be free from scattered materials, maintain good housekeeping at the entire work area.
• Ensure to wear Full Body Harness when working 1.8 meters above ground and observe 100% tie-off
• Position the body opposite air direction during opening the valve.
• Install barricade and post warning signs such as “NITROGEN CLEANING IN PROGRESS”.
• Assigned dedicated standby man outside the danger zone to ensure no personnel will enter the area where nitrogen release is in progress.
• Material Safety Data Sheet (MSDS) must be in record and available at site.
