JOB SAFETY ANALYSIS FOR FIRE ALARM SYSTEM AND GAS DETECTION DEVICES

RISK CONTROL MEASURES

1. Obtained proper permit for the activity

 

·         No potential incident associated with the activity

 

·         Electrical Manager  will issue the permit for the activity

·         Work Permit System should be followed as per GI 2.100, CSM 1-4

 

 

2. Installation of fire alarm system ·         Incompetent personnel

·         Lack of  trainings, knowledge and experienced

·         Defective tools

·         Hand cutting and laceration

·         Poor communication

·         Electric shock and electrocution

·         Fire and explosion

 

 

·         All personnel must attend the TBM – Tool Box Meeting & TSTI- Total Safety Task Instruction to be conducted by electrical supervisor/foremen/safety staff

·         Personnel must be trained, experienced and competent for the installation techniques of fire alarm system

·         Trained and competent personnel of using suitable tools will carried out cable termination

·         Permit to work system must be followed and LOTO system

·         Use the right tools for the jobs

·         All defective hand & power tools must be repaired or remove from the site and put tag (not in service)

·         Install barricade with signage properly on working area

·         Provision of  two way radio for the good communication

·         Dispose waste materials and segregation properly

·         Fire extinguisher and first kit must be available

·         Wear complete P.P.E. at all times

3. Installation of supports, welding, cutting, drilling ·         Pinch point

·         Cut and laceration

·         Short circuit/ and electric shock

·         Fire and explosion

·         Burn

 

·         Only certified welders must do the welding jobs

·         Ensure that all tools are inspected with color code by competent electrician  and proper maintenance

·         Inspect all extension cables and never overload

·         Ensure that all electrical panels and breakers are proper grounding

·         If needed, ELCB / GFCI should be installed on distribution panels

·         Install adequate lightings at night or dark areas

·         Proper positioning of the body and watch out in pinch point location

·         Use the right tools for the jobs

·         Always focus to the works and be presence of mind

·         Remove all combustible and flammable material around the hot work activity

·         Proper usage of P.P.E. (face shield, apron, etc.)  for welding, cutting, drilling at all times

·         Sufficient fire extinguisher , blankets and fire watch man

·         In case of fire emergencies, isolate the power before tending the fire or call for rescue

·         Apply first-aid and call medical assistance

·         Fire doors shall be kept closed always to control spreading of fire

·         Use the fire extinguisher to extinguish the fire and if the fire continuously  and call emergency

4. Working at elevated working platform and scaffold platform  ·         Fall

·         Drop

·         Scaffold inspector or supervisor must be ARAMCO certified

·         Ensures the types of scaffolds shall be designed, constructed, inspected, tagged and used per GI 8.001 and CSM II-2 scaffolding.

·         Tower shall be erected by trained and competent persons and should be stable with maximum height to base ratio not exceeding 3:1 for use

·         Falling protection will be established

·         Ladder will only be used for access and not for working platform and will be tied or footed at all times when used

·         Scaffold working platform will be inspected by a competent person with updated scaff tag

·         All workers shall be wear full body harness at more than 1.8 meters from ground level and 100% tie off required

·         Use tool bag, rope with pulley to transfer of tools and materials from ground level to the top level.

·         Never taking shortcuts to transfer in deferent platform

·         Apply 3 point contacts when climbing or going down of ladders and always facing the ladder

5. Moving platform (man lift) ·         Striking object

·         Tip over

·         Struck by falling object

·         Lift overturn

·         Fall

·         Fitted with PWAS

·         The mobile equipment shall be certified by 3rd party agency 

·         Operator must be ARAMCO certified.

·         Any mobile equipment which is taken to be platform shall be stored in safe manner and out of access/egress routes.

·         Red and white warning tape shall be used to highlight any objects which protrude into access/egress routes

·         Do not exceed vertical or horizontal reach limit or

·         The loading capacity of the lift

·         Keep all workers and unauthorized person away from the raised boom

·         Materials must be secured properly from the bucket

·         Always setting the brake, engage the power or lower the out rigger

·         Keep both feet on the floor of the man lift basket

·         Weight of the operator and tools should not exceed the designed load limit set by the manufacturer

·         Do not lean over the man lift basket or reach too far or you may lose your balance

·         Full body harness should be hooked to the anchor point specified on the operator’s manual and 100% tie off required

·         The movable material on the moving platform shall be fixed or removed.

6. Painting operation

(touch up paint)

·         Fume, mist inhalation, skin burn, eye irritation

·         Fire emergency

·         Toxic atmosphere

·         Burn & injury

·         Trained and certified personnel of painting should perform the job

·         Avoid inhalation, skin contact, spray under well ventilated condition

·         Working areas shall be well ventilated

·         Clean air-mask frequently

·         If your eyes were accidentally comes in contact with harmful chemicals, your eyes should be cleaned with clean water 10 to 15 minutes, if still irritated, contact medical assistance immediately

·         Paints, thinners are to be keep away from fire or flame

·         In case of fire emergency, PCP fire extinguisher are to be used

·         Recue victim and applied first-aid and call medical assistance

·         Thinners, paints shall be stored at well ventilated, open and covered shelter.

·         Provide SDS – safety data sheet in workplace

 

7. Working in confined space area ·         No Confined Space Entry PTW and training

·         Hazardous atmosphere

·         Poor illumination

·         Oxygen deficiency/enrichment

·         Poor communication

·         Provide confined space  entry (CSE) PTW as per GI 2.100, CSM 1-4, work permit system

·         Provide gas test with competent person as per GI 2.709, gas testing procedure,  CSM 1-6 confined spaces

·         Isolate all lines connected with confined space area, put Lockout and hold tags as per GI 6.012

·         Ensure all personnel are trained and certified

·         Designated hole watch man with confined space log sheet (IN/OUT)

·         Provide barricade and safety signage (confined space area and gas test required)

·         Provide ventilation fan and lights

·         Provide safe access & egress

·         Constant communication between the entrant and standby man/ hole watch man

·         Provide rescue equipment

·         Wear complete required P.P.E. at all times as per

8.  

Manual handling

·         Drop

·         Cuts

·         During manual lifting of materials, the body shall be straight up, not bent

·         Shall not exceed the lifting capabilities

·         Suitable P.P.E. shall be worn to protect feet and legs, load shall be secured prior to lifting

·         A safe work practice shall be implemented to reduce the possibilities of accident caused by lack of communication

·         Personal Protective Equipment (PPE) shall be worn at all times.

9. Housekeeping ·         Trips, slips, falls ·         Removed unnecessary materials around the working area

·         Properly disposal of waste materials after the activity

·         Covered open holes and put signage

10. Weather condition

 

·         Working direct

·         Sun light heat stroke.

·         Should ensure that all labors should be provided cool water and shelter avoid such condition (glucose powder to be added rather salt tab.) 

·         Should maintain and give at least 15 mins. Rest for those mechanic working direct sunlight to avoid dehydration

·         Employees to try to work and take breaks in the shade. Be aware that sunburn can occur even when in the partial shade or when cloudy.

·         Employees should drink plenty of water to avoid dehydration

 

·         Poor visibility cause by fog and sand storm ·            Should stop the work if poor visibility to avoid any accident of

damage to property

 

 

 

·         Wet cause by rain and water cleaning

 

·         Slippery

·         Ensure the electrical panel are sealed and covered

·         Inspect  the equipment and electrical before starting the activity
