JOB SAFETY ANALYSIS JSA FOR INSTALLATION OF CABLE TRAY

ACTIVITY 

Transportation and Handling of cable Trays to designated work location.

 

HAZARDS

Falling material from vehicle or trucks.  

Traffic accidents.

PREVENTIVE ACTIONS

Experienced & Trained rigger should load and unload the materials.

Equipment and vehicle must be operated by qualified & certified operators.

Rigging tool like sling, lifting belt, D-shackles etc. must be inspected.

Keep speed on its required limit.

Equipment or vehicle should be in good condition & checked regularly by HEQ department.

Qualified signal man to guide driver to intended work location.

Drilling works

Electric shock.

Twisting of drill machine that might cause injury.

Ensure that no open wires and splicing joints on extension cords.

Equipment shall be inspected and color coded.

Socket outlets shall be weather proof industrial type and approved as per project requirement provided with grounding terminals.

Drill machine shall be in good running condition.

Used appropriate PPE; Leather gloves.

Drill machine must be provided with guide stopper, properly positioned and grip.

Welding works

Fire Hazard.

Hot Work permit.

Fire watch required.

Eye injury.

If welding works are done in a classified area near existing plant (Q-Chem I) or
within existing plant approved fire box must be provided.

Equipment shall be properly grounded and fire extinguishers are in place.

Compressed Gas Cylinders must be kept far enough away from welding so that sparks and hot slag will not reach the cylinder.

Wear approved welding mask.

Cable Tray & Support installation.

Cuts and Laceration.

Muscle Strain/Sprain.

Proper tools and equipment shall be used when installing cable tray & supports. Inspected and in good working condition.

Proper body mechanics while manual lifting cable tray, body should be straight & knees bent. Use two persons on lifting.

Personal Protective Equipment (gloves and goggle) must be worn while work is in progress.

Unauthorized personnel entering worksite.

Danger from falling objects.

Being hit by crane

Proper barricade shall be executed so as to prevent entry of unauthorized personnel entering worksite.

Wearing of Personnel Protective Equipment shall be used at all times.

 

The area of barricade shall cover the entire rotational path of the crane.

 

Safety warning signs shall be visible and clear at work area.

Use of working platform.

Danger from falling,.

Scaffolding shall be inspected by trained and certified scaffolding inspector and green tagged. 

Materials kept on platform should be as per the load bearing capacity of the platform. 

Only trained personnel or workers are allowed to work on a high level or elevation platform.

Full body harness shall be worn at all times while working on platform. Lanyards must be properly hooked (100% tie off).

No tools shall be carried by hands while climbing on a working platform.

No stepping on handrails.

Good housekeeping shall be maintained above working platform.

Cranes and general lifting operation.

Malfunction in the operation of the crane or any lifting machine.

Danger in using underrated lifting equipment and accessories.

Inspection must be conducted and recorded daily by operators and mechanic to ensure that the machine is in good condition. Only trained and certified operators are allowed to operate the equipment.

Cranes and other lifting equipment shall be positioned and operated only in a plane surface and stable ground. 

Lifting accessories such as nylon slings, wire ropes and shackles shall be properly selected according to the weight of object being lifted. Slings inspect for tears, cuts, snags etc.

Winch operation.

Being caught by winch machine pulley/cable wire.

Danger from falling of cable tray while lifting.

Only qualified winch operator are allowed to operate the machine.

Winch machine cable wire path shall be properly guarded or barricade.

Winch machine shall be properly fastened and rest on a solid platform.

Winch machine shall be properly operated slowly without jerking.

Proper coordination and signals between the winch operator and rigger while cable tray is being lifted.

No person should be directly under the load during lifting. Barricades installed around proper work radius.
