Lockout / Tagout Steps – Electrical

 

The following attachment provides clarification or site specific requirement in regards to the Lockout/Tagout Global Standard.

3.             SUMMARY (Site Specific Details)

The assessment referenced in this section of the global standard will be carried out annually by the EH&S Manager to ensure ongoing compliance and effectiveness of the procedure.

 

4.1.1     Preparation for Shutdown (Site Specific Details)

 

Facility operator(s), Process Coordinator, and Maintenance Production Coordinator are responsible for equipment at the facility.  They must determine the hazard(s) both to personnel and equipment, as necessary, by reviewing current drawings, P&IDs, specifications, and by experience or other means, and by a physical inspection of the work site. This person can then determine the valves to be closed or opened, and the electrical switches to be opened, etc., to provide a safe working environment.

 

 

4.1.2     Shutdown of Equipment/Energy Isolation

 

Being responsible for the equipment, operators will always install their lock first and remove their lock last.

 

It is the practice at the Paulsboro plant to lock open the bleed for all energy isolations involving a double block and bleed arrangement.

 

4.3.1     Lockout Requirements

 

Per the procedure sites will have a system in place to manage locking devices that are used for isolation purposes.  This system shall ensure that one type/model of lock is used for one purpose and a distinct type/model is used for other purposes. All site personnel and contractors shall be aware of this system. Distinct locks should be used for the following:

 

  1. Permit locks.

 

  1. Personal and group locks.

 

  1. Other locks, e.g., process, operational, or utility locks.

 

To accomplish this requirement, the Paulsboro plant will utilize the following system:

 

  • Production department locks will be designated with blue color markings.

 

  • The Maintenance and Instrumentation Departments (including Electricians) will have personal locks of a different color than Production locks with the mechanic’s name clearly identified on it. Red is the designated color for the maintenance department. A single key for each lock is to be kept only by the maintenance employee after the lock is installed. Locks used for isolation of energy must never be used in any other service. No other employee but the one installing the lock will have a key to the installed lock or access to the lock’s key.

 

  • The Production department will use a yellow utility lock for long term lock outs until the equipment is removed or the hazard is removed. The Breakers will maintain the lock box with an active list of these locks and locations in the plant.

 

  • The various department heads will ensure that an ample supply of locks and tagging equipment are available for any task which requires lock, tag, and try.

 

 

4.3.3     Removal of Personal Locks or Tags When Individual Is Not on Site

 

If unable to contact the person whose lock must be removed, the job supervisor must get the Plant Manager to authorize removal of the lock.
