SEQUENCE OF BASIC JOB STEPS

POTENTIAL HAZARDS

REQ HAZARD CONTROLS/SAFE WORK PROCEDURES 

• Joint Site Inspection, Planning and Prepare the site for work • Unauthorized work
• Unwanted/loosed materials can cause personal injury.
• Workers have no idea of the job on hand.
• Unauthorized entry
• Presence of hazardous atmosphere.• Injuries can occur to the worker’s expose body parts.
• Permit Issuer and permit receiver conduct the job site inspection.
• HSE Department Take gas test.
• Provide caution tape , barricade around the area.
• Use Proper PPE.
• Provide certified/Trained Fire watch.
• Provide fire extinguisher.
• All heavy equipment Shall be inspected and approved by HSE Department.
• All Equipment’s operators shall be certified by Saudi Aramco and record shall be submitted to HSE Department for approval.
• Follow permit to work procedure and obtain applicable PTW for vehicle access.
• Heavy Equipment’s must be fitted with PWAS
• Personnel in the area should be aware of potential hazards.• Barricade the area with posted signs to parts. restrict unauthorized personnel from coming in
• Close coordination meeting shall be held daily.
• Approved work permit shall be available before proceeding with the job. During construction period work permit system will be applied.
• Job supervisor shall ensure that housekeeping has been conducted on the area prior to starting activity.
• TSTI shall be carried out by the job supervisor emphasizing the hazards identified stated on the permit.
• TSTI ensure must be understand by all workers
• Personnel assigned to the tasks shall be experienced workers.
• Tools and equipment preparation and inspection. • Using damage or fabricated tools can cause physical injury.
• Can cause physical injuries when used without proper inspection and certification equipment.

Prior to work all tools shall be inspected for safe
usage. Damage tools are to be removed from
site. Fabricated tools are not to be used. Use
only tools fitted for the job.
• All tools shall be inspected before using and proper color coded. Equipment’s shall be certified as per SA standards.
• Prior to its operation as a minimum requirement, operator shall inspect his equipment following checklist provided by the HSE Department.
• Handling of Air Fin Fan Material • Improper handling of material resulting in injury or health risks.
• Slips Trips or Falls
• Handle all material with care and arranged properly. Ensure equipment will be inspected by HSE Department and set properly.
• Use appropriate hand, face, feet, and body protection equipment.
• Fin Fan Cooler material to be kept in proper place.
• Ensure all material kept out to prevent unnecessary tripping hazard.
• Erection of Air Fin Fan Cooler  • Unstable grounds/ strong winds.
• Crane collapse which may lead to personnel injury or damage to property.
• Crane mechanical failure.
• Unauthorized use of the equipment.
• Crane operator and riggers incompetency And not certified.• Unsecured loads during lifting.
• Check wind velocity prior to lifting and ground stability for outriggers. Fully extend outriggers with wood matting prior to lifting.
• All Equipment’s, Cranes operators Man Lift and riggers shall be certified by Saudi Aramco and record shall be submitted to HSE Department for approval.
• All rigging activities perform by SA approved Riggers as per load Categories.
• All cranes shall be levelled in accordance with manufacturer’s specifications and shall be located on a properly compacted foundation prior to performing any lift.
• During a lift, the crane’s outrigger shall be fully extended and lowered to raise the wheels and level the crane, unless otherwise specified in the crane manufacturer’s load chart.
• Outrigger mats shall be used under all crane outrigger float pads during crane lift. It shall be designed to support the total weight of the crane load to be lifted. Mats shall not be permanently secured to the
outrigger pads.
• Riggers to be assigned length of boom to be used as radius of barricaded area on the ground. 
• Work at Height • Fall of personnel from height.
• Scaffold collapse.
• Fall of material from height.
• Defective tools.
• Falls through fin fans.
• Safety training work at height shall be attended from HSE Department..
• Full body Harness shall be strictly required when working at height and 100+ tie off.
• All usable materials shall be secured proper protection to avoid from falling objects.
• Scaffold shall be erected under the supervision of SA approved Scaffold Supervisor
• Scaffold inspection and tagging shall be implemented as per SA standard.
• Working in adverse weather condition or sandstorm • Personnel may suffer heat stress related sickness during hot season.
• Dehydration
• Sand particles can cause eye irritation
• Job supervisor shall make sure that regular supply of water is available and complete checklist and record shall be maintain by HSE department.
• Frequent rest cycle to be implemented when necessary.
• Cold water and drinking cups shall be available all the time.
• Safety goggles or safety glass with side shields shall be used.
• Stoppage of work if bad weather conditions persist
• Housekeeping • Unwanted materials, debris and tools can cause tripping, slipping hazard.

• Scattered Materials and accumulation of hot

• Regular housekeeping shall be performed.
• Adequate and correct stacking of materials.
• Regular inspection and immediate corrective action.

NOTE: THIS JSA SHOULD BE DISCUSSED AND EXPLAINED TO THE ENTIRE EXECUTION TEAM.
